All of the forms with the symbol can be downloaded to your computer in Portable Document Format (PDF). You will need Adobe Acrobat Reader in order to read these documents. Many computers come pre-loaded with Reader and will work with no additional software.

If your computer is not already equipped with a copy of Acrobat Reader, this software is available free on the web.  After you have downloaded and installed the software, simply click on the document you want, and in a short while,  you will have the full report on your desktop.
